Abdominal Aortic Aneurysm (AAA) Surgery


Definition/Overview An Abdominal Aortic Aneurysm (AAA) is an enlargement or weakened bulging area of the aorta (the main artery that caries blood away from the heart). As it enlarges, it can rupture and cause death. Depending on the patient’s general health, the size of the aneurysm and the rate of growth, surgery may be recommended. Procedure The traditional method of repairing an abdominal aortic aneurysm is open repair, which is performed under general anesthesia. The surgeon begins with a large incision to the abdomen. Blood flow is temporarily stopped above and below the aneurysm. The aneurysm is opened and repaired by sewing a synthetic tube known as a graft in the bed of the enlarged aorta. After the procedure, you will be monitored closely in the intensive care unit (ICU). You may have a tube in your throat until you are stable enough to breathe on your own. After this tube is removed, you will be asked to cough and take deep breaths. Though this will be painful, it is crucial so as not to develop pneumonia. You will be given pain medication as needed. When your physician determines you are ready, you will be moved to a post-surgical nursing unit where you will gradually start walking and taking in solid foods. You may be in the hospital for 4 to 7 days, but depending on your circumstances, recovery may be 6 weeks to 3 months.
Angioplasty

Definition/Overview Angioplasty is a minimally invasive procedure performed to open blocked arteries. It is used when blood flow in one or more of your coronary arteries is restricted. This restriction, if left untreated, can lead to chest pain, possible heart attack or stroke. Procedure Angioplasty takes 1 to 2 hours and is performed at a hospital or surgical center. Though you will be awake during the procedure, you will receive medication to help you relax. The site where the catheter will be inserted is numbed, and imaging techniques are used to guide a balloon- tipped catheter into the artery to be treated. Once there, the balloon on the end of the catheter is inflated to break down the plaque and/or push it against the artery wall. Then the balloon is deflated. This is repeated several times until satisfactory results are achieved. At this point, the catheters are removed and blood flows freely through the artery. During the procedure, the IV sedative may make you relaxed and sleepy. You may feel slight pressure when the catheter is inserted and mild discomfort when the balloon is inflated. This should lessen as the balloon is deflated. You will remain in recovery for several hours during which time your catheter site will be checked for bleeding or swelling, and your vitals will be monitored. Your physician may prescribe medication prevent blood clots. After you return home, you will be instructed not to lift heavy objects or exercise strenuously for 24 hours. You should avoid smoking on a permanent basis.

Carotid Endarterectomy

Definition/Overview Endarterectomy is the surgical removal of plaque from an artery that has become narrowed or blocked. This procedure is commonly recommended for those who have had a transient ischemic attack (TIA) or a stroke resulting from a narrowing of more than 70% of the carotid artery. Procedure During this inpatient procedure, you will either be given general anesthesia or your surgeon will numb the area over the artery and you will remain awake for the surgeon to monitor your brain function (due to the decreased blood supply). Through a small incision in the neck, the affected carotid artery is exposed and opened to allow your surgeon to remove the plaque causing the narrowing. During the repair, blood flow is redirected around the narrowed area using a shunt. If necessary, a vein patch or synthetic patch may be sewn to the carotid artery opening to widen the artery. After the shunt is removed, the artery and incision are closed. This procedure is typically completed in about 1 to 2 hours. Patients with uncomplicated cases are usually discharged from the hospital 48 hours after surgery.

A Femoral endarterectomy is the surgical removal of fatty deposits, called plaque, from the walls of arteries other than those of the heart and brain. The surgery is performed when plaque blocks an artery and obstructs the flow of blood and oxygen to other parts of the body, most commonly the legs but also the arms, kidneys, or intestines. The peripheral arteries most often treated with endarterectomy are those that supply the legs, especially the aortoiliac arteries in the pelvic area. Other arteries that may be treated with endarterectomy include the femoral arteries in the groin, the renal arteries that supply the kidneys, and the superior mesenteric arteries that supply the intestines.

Endarterectomy surgery is performed to treat advanced peripheral arterial disease (PAD). PAD most often occurs as a result of atherosclerosis, a condition characterized by the gradual build up of fats, cholesterol, cellular waste, calcium, and other substances on the inner walls of large and medium-sized arteries. Plaque, the hardened, waxy substance that results from this build up, can cause narrowing (stenosis) of an artery and block the flow of blood and oxygen. Peripheral endarterectomies are performed to reopen blocked arteries and to restore blood flow in the body (revascularization), helping to prevent heart attack, stroke, the amputation of a limb, organ failure, or death.

Definition/Overview An Abdominal Aortic Aneurysm (AAA) is an enlargement or weakened bulging area of the aorta (the main artery that caries blood away from the heart). As it enlarges, it can rupture and cause death. Depending on the patient’s general health, the size of the aneurysm and the rate of growth, surgery may be recommended. Procedure The traditional method of repairing an Abdominal Aortic Aneurysm begins with a large incision to the abdomen. However, a newer — yet widely accepted — procedure involves the placement of a stent graft. Using a small groin incision and possibly a small abdominal incision, catheters loaded with the tube graft are passed through the groin arteries into the abdomen. The stent graft is deployed under X-ray guidance. The tube graft occludes the aneurysm at the top of the normal aorta and at the bottom of the normal aorta. Following the procedure, the patient may or may not be taken to the intensive care unit (ICU) or a post anesthesia care unit (PACU). Vitals are monitored and you will eventually be transferred to a regular nursing care unit. You will be given pain medication if you have not had an epidural during surgery to alleviate postoperative pain. Gradually you will be encouraged to walk and take in solid food as tolerated. The initial recovery time is usually 2 to 3 days, with patients returning to normal activities within weeks. Long-term follow up with serial CT scans is required.
Peripheral Artery Bypass

Peripheral Artery Bypass


Definition/Overview This procedure, sometimes referred to as a peripheral vascular bypass or a lower extremity bypass, is performed to treat Peripheral Artery Disease, when the buildup of plaque in an artery has blocked the normal flow of blood that carries oxygen and other nutrients to the lower extremities. Procedure This is an open procedure requiring general anesthesia. The surgeon makes an incision to expose the affected artery above the blockage, and another to expose it below the blockage. The arteries are blocked off with vascular clamps, and the surgeon creates a new path for blood to flow around the blocked or narrowed artery. The new path — or bypass — is created from either another blood vessel from the patient’s leg or from a synthetic (manmade) graft, which is sutured (stitched) into an opening in the side of one artery then into the other. The clamps are removed and the flow of blood is observed to make sure that it bypasses the blocked portion of the affected artery. Recovery instructions will depend on the area where the bypass was performed. The approximate hospital stay is two to four days, and the patient will be given specific instructions on activity restrictions and lifestyle modifications

The skin is a barrier to the outside world protecting the body from infection, radiation, and extremes of temperature. There are many types of wounds that can damage the skin including abrasions, lacerations, rupture injuries, punctures, and penetrating wounds. Many wounds are superficial requiring local first aid including cleansing and dressing. Some wounds are more severe and need medical attention to prevent infection and loss of function, due to damage to underlying structures like bone, muscle, tendon, arteries and nerves. The purpose of medical care for wounds is to prevent complications. While important, cosmetic results are not the primary consideration for wound repair. Animal and human bites should always be seen by a medical professional because of the high rate of infection. Please call 911 if you have either one of these conditions and need immediate help. Most wounds can be treated at home with routine first aid including thorough washing and dressing to prevent infection. You may be required to visit our offices at Peoria Surgical Group to meet with a physician or physicians assistant to set a plan of action for your wound care. Some of the following are reasons medical care should be obtained for a wound: If the wound is due to significant force or trauma and other injures are be present. If bleeding cannot be stopped even with persistent pressure and elevation. If there is concern that wound requires repair with sutures (stitches). The size and location of the wound are important considerations. Most facial wounds may need to be repaired for cosmetic reasons, especially if they involve the lip or eye. If the wound is caused by an animal bite. At least 50% of dog bites, 80% of cat bites, and 100% of human bites become infected. There is also a need to consider rabies immunizations if appropriate. If the wound is very dirty and cannot be easily cleaned. If there is evidence of infection including redness, swelling, increased pain, and pus at the wound. If tetanus immunizations are not up to date, then a booster is needed within 48 hours. If the patient has never been immunized, the initial tetanus prevention with immunoglobulin should be given immediately.
Appendectomy

Appendectomy


Definition/Overview An appendectomy is the surgical removal of the appendix. The procedure is normally performed in an emergency situation, when a patient is suffering from acute appendicitis. Appendicitis is an inflammation of the appendix. If left untreated, an inflamed appendix could eventually burst, or perforate, leaking infectious material into the abdominal cavity, eventually causing sepsis. Procedure: Once the diagnosis of appendicitis has been made (usually based upon a physical exam, blood and urine tests, and possibly an X-ray, ultrasound or CT scan), antibiotics are administered prior to surgery. Appendectomies can be performed using an open method, or laparoscopically. In an open appendectomy, a two to three inch incision is made through the layers of the abdominal wall near the appendix. After examining the surrounding area, the appendix is removed by cutting it from the colon and sewing over the hole. The incision is then closed. This procedure is performed while the patient is under general anesthesia, and the hospital stay is usually one or two days. If the appendix has been perforated, the surgeon may insert a drain to release fluid from the abscess before removing the appendix. In this case, antibiotics must be administered after the surgery and the patient may remain in the hospital for a longer period of time. The laparoscopic technique involves making a series of small incisions in the abdomen. One incision is for insertion of a laparoscope, a thin telescopic instrument attached to a video camera that allows the surgeon to inspect the abdomen and appendix. Special instruments are inserted in the other incisions, and the appendix can be removed through one of the small incisions. This approach generally results in less post-operative pain for the patient and a quicker recovery time. Laparoscopic surgery also enables the surgeon to make a clear diagnosis in case appendicitis is in doubt. General surgery, despite its name, is a surgical subspecialty that primarily focuses on abdominal organs and the intestines including esophagus, stomach, colon, liver, gallbladder and bile ducts, and often the thyroid gland (depending on the availability of head and neck surgery specialists). This encompasses a wide range of procedures including appendectomies and hernia repair to lung volume reduction surgery and thyroidectomy. Definition/Overview A cholecystectomy is the surgical removal of the gallbladder. Although it is occasionally performed for gallbladder cancer (which is very rare), it is most often used to treat symptomatic gallstones. Procedure In an open cholecystectomy, the patient is given general anesthesia and the gallbladder is removed through a 4 to 7 inch incision. Patients usually remain in the hospital overnight and recovery may take several weeks at home. A laparoscopic cholecystectomy involves making several smaller incisions in the abdomen and inserting a laparoscope — a thin, lighted instrument with a camera on the end — into the abdominal cavity. The surgeon is able to view images on a video monitor, and performs the operation by manipulating the surgical instruments through the other incisions. The gallbladder is removed through one of the incisions. This procedure is also done under general anesthesia. However, it generally involves less pain, quicker healing time and fewer complications. Most patients are discharged the same day, and can return to a normal routine in about a week. Your surgeon will determine if you are best suited for the open or laparoscopic technique.

Definition/Overview A liver resection is the surgical removal of a portion of the liver, and is usually performed in order to extract various types of liver tumors. The liver is the only organ with the ability to regenerate itself. Up to 75% of the liver can be removed as long as the remaining tissue is healthy. Procedure: A liver resection is performed under general anesthesia. An incision is made in your abdomen below the rib cage. The surgeon will cut the section of the liver to be removed from the supporting veins, arteries and bile ducts. The incision is then closed using staples or stitches. The hospital recovery time for a liver resection can be up to a week or more. You will experience some discomfort during recovery, and your physician will prescribe pain medication during your recovery.

Definition/Overview A pancreatectomy is the surgical removal of the pancreas. It is the most effective treatment for cancer of the pancreas, and can also be performed due to injury or trauma to the pancreas, or chronic pancreatitis (continued inflammation of the pancreas that can result in permanent damage to the organ). Procedure: The surgical procedure you are having will determine the length of your hospital stay as well as your recovery period. All of the following procedures require general anesthesia. Central Pancreatectomy This procedure is performed if there is a benign tumor in the neck of the pancreas. Since this is a difficult area of the pancreas to reach, the goal is to remove any tumors in the area while preserving as much of the pancreas as possible, as well as avoiding complications such as diabetes and malabsorption of nutrients. Distal Pancreatectomy This procedure is performed if there is a tumor present in the body or tail of the pancreas. The bottom half of the pancreas is removed and the cut edge is sutured to prevent leakage of pancreatic juice. In some cases, distal pancreatectomy is performed in conjunction with a splenectomy. The spleen is removed with the pancreas since the blood supply to the spleen is associated directly with that of the pancreas. Subtotal and Total Pancreatectomy Pancreatectomy is a procedure to remove part or the entire pancreas. A partial removal is called a subtotal pancreatectomy; a removal of the entire organ is called a total pancreatectomy.

Definition/Overview A panendoscopy is a procedure that a physician uses to examine the lining of the esophagus, stomach and duodenum (the first part of the small intestine). A panendoscope is a long, flexible tube that is passed through the mouth and back of the throat into the upper digestive tract. This procedure is commonly performed since many problems of the upper digestive tract can’t be diagnosed with an X-ray. Procedure: You will be given medication to make you relaxed and sleepy, and the back of your throat may be sprayed with a local anesthetic. The panendoscope will be inserted through your mouth and each part of the esophagus, stomach and duodenum. This procedure is performed on an outpatient basis. Your throat may be sore for a few hours, and you may feel fullness in your abdomen because of the air that was introduced to examine your stomach.

Definition/Overview The spleen is an organ located in the upper left part of the abdomen near the stomach, and is the largest lymphatic organ in the body. The spleen filters and serves as a reservoir for blood and destroys aged blood cells. Procedure: Splenectomies can be performed using an open method, or laparoscopically. Splenectomies can also be complete or partial. A complete splenectomy is the removal of an enlarged or ruptured spleen. This procedure is performed under general anesthesia. If the spleen is enlarged or ruptured, the surgeon makes a cut in the abdomen and ties the splenic artery off to prevent blood loss and reduce the spleen’s size. He then detaches the ligaments holding the spleen and removes, and sends tissue samples for analysis. In a partial splenectomy, the surgeon removes only a portion of the spleen. In some cases, this can reduce the pain of an enlarged spleen and reduce the incidence of infection. Laparoscopic splenectomy is the removal of the spleen using several small incisions. The surgeon inserts a thin, lighted scope with a camera on the end into one of the incisions, and the surgical instruments in another. Laparoscopic splenectomy reduces pain, scarring and length of hospital stay. Your physician can evaluate your particular case and determine if you are a candidate for laparoscopic surgery. Hospitalization time for a splenectomy is about a week if the procedure is done through an abdominal incision. If it is done laparoscopically, hospital time may be reduced.

Definition/Overview A hiatal hernia occurs when the normal opening in the diaphragm is too large. If left untreated, the stomach or other abdominal contents may bulge, or herniate, into the chest. This can cause heartburn, pulmonary aspiration due to the displacement of a lung by the hernia, or GERD (gastroesophageal reflux). There are two types of hiatal hernias — sliding and para-espohageal hernias. In a sliding hiatal hernia, part of the stomach moves up through an opening (hiatus) in the diaphragm. This is the most common type of hernia. A para-esophageal hernia occurs when the stomach twists upon itself and moves into the chest along the esophagus. Patients may have difficulty swallowing or have reflux symptoms. Procedure: Sliding hiatal hernias may not require treatment unless symptoms become problematic. Treatments generally include making changes to lifestyle and eating habits, and if necessary, taking antacids, acid reducers or acid blockers. A hiatal hernia may lead to GERD, in which case it will be treated with prescription medication, and if necessary, surgery. Para-esophageal hernias require a surgical procedure involving moving the herniated area of the stomach back into the abdominal cavity. In an open hiatal hernia repair a cut is made in the stomach area while the patient is under general anesthesia. The esophageal hiatus (the opening in the diaphragm through which the esophagus passes from the chest into the abdomen) is tightened, making it smaller so the stomach can no longer bulge through. The esophagus is then attached to the diaphragm. In some cases, this procedure can be performed laparoscopically, using a thin, hollow tube with a camera on the end. Surgical tools are sent through the laparoscope to repair the hernia. This procedure results in fewer and smaller incisions, less pain and scarring, and a shorter hospital stay.

Definition/Overview A hernia is a tear or weak spot in the abdominal wall. It occurs when an organ or tissue squeezes through a hole or a weak spot in a surrounding muscle or connective tissue called fascia. Hernias are classified as inguinal, incisional, femoral and umbilical. Procedure: A herniorrhaphy is performed for individuals who are suffering pain or other symptoms of a hernia. Although the surgery performed to correct inguinal, incisional, femoral and umbilical hernias each differ slightly, the basic procedure is very similar. In an open hernia repair, a single, long incision is made in the groin. If the hernia is bulging out of the abdominal wall, it is pushed back into place. If it is going down the inguinal canal, the hernia sac is either pushed back or tied off and removed. In some smaller hernias, the weak spot in the muscle wall is repaired by sewing the healthy muscle tissue together. In some cases, mesh patches of synthetic material are sewn over the weakened area in the abdominal wall after the hernia is pushed back into place, decreasing the risk of hernia reoccurrence. In a laparoscopic procedure, a thin, lighted tube with a camera is inserted thorough a small incision made under the patient’s belly button. Two other small incisions are made in the lower abdomen where the instruments are inserted. By viewing a TV monitor, the surgeon pulls the hernial sac back into the abdominal cavity, exposing the defect. He then covers the weakened portion with a mesh patch. The open hernia repair is performed while the patient is under general anesthesia, but the laparoscopic may be performed either under general anesthesia or, in some cases, regional anesthesia combined with a sedative.

Definition/Overview A lumpectomy is a surgical procedure performed to remove a suspected malignant (cancerous) tumor or lump along with a small portion of the surrounding tissue from the breast. How much of the breast is removed during the procedure is dependent on the size and location of the lump. The characteristics of the specific cancer are also a factor when determining if a lumpectomy is appropriate. Women in the early stages of breast cancer are often considered good candidates for this procedure. Procedure Prior to a lumpectomy, the physician may have performed a fine-needle biopsy of the breast. If the mass cannot be located by touch, a wire-localization procedure will be performed, which involves inserting a fine wire and utilizing ultrasound to confirm the tumor’s location. Either local or general anesthesia may be used, depending on the extent of the surgery. The surgeon will make an incision over the targeted malignancy and remove the tumor, along with a small layer of surrounding tissue. If necessary, the physician will make a separate incision near the underarm to sample or remove axillary lymph nodes, which are tested for cancerous cells. A drainage tube may be inserted during the surgery to remove fluids, and the wound is closed with dissolving stitches. This procedure typically takes 1 to 3 hours. You will spend a short time in recovery, and most patients are able to go home the same day. Depending on the extent of the lumpectomy, many women can usually resume normal activities within 1 to 2 days.

Definition/Overview A mastectomy is a surgical procedure in which most or all of the breast tissue is removed, in order to treat breast cancer. There are several types: Simple (Total) Mastectomy The removal of all of the breast tissue, but none of the underlying muscle or lymph nodes. Modified Radical Mastectomy The removal of all of the breast tissue and lymph nodes on the same side of the body as the breast. Radical (Halsted) Mastectomy The removal of all of the breast tissue, the underlying muscle, and the lymph nodes on the same side of the body as the breast (rare). Skin-sparing Mastectomy The removal of the breast tissue, but breast skin is kept for immediate breast reconstruction. Procedure This procedure is performed under general anesthesia. The surgeon will make a curved incision (that follows the natural curve of your breast) closes to the area of the tumor. If the tumor can be seen and/or felt, she will remove the tumor as well as the ring of healthy tissue surrounding it. If it cannot be seen or felt, you will have a procedure before the surgery that will locate and mark the tumor by either use of a mammogram or ultrasound. After the tumor has been removed, a drain may be placed to collect excess fluids. The incision will be closed. The actual mastectomy procedure takes approximately two to three hours; however, if you have chosen to have reconstructive surgery at that time, the length of your surgery will increase. Depending on your circumstances, you may be able to return home the same day.

Definition/Overview A needle biopsy is a diagnostic procedure that involves taking a small sample of breast tissue from an area that appears suspicious from a mammogram. Procedure During this procedure, you will be given a local anesthetic to numb the area. The physician will use a special, hollow needle to extract cells from the suspicious area for further diagnosis. This procedure is most often performed on tumors that can be felt through the skin, such as suspicious breast lumps and enlarged lymph nodes. Types of needle biopsies include: Fine-needle Aspiration A long, thin needle is inserted into the suspicious area and a syringe used to draw out fluid and cells for analysis. Core Needle Biopsy A larger needle with a cutting tip is used to draw a column of tissue out of a suspicious area. Stereotactic Biopsy This is performed in order to get tissue from a lump that cannot be felt during the breast exam. The surgeon will use a special type of X-ray imaging to find the lump that the needle must target in order to get an accurate tissue sample. The needle will follow the X-ray to the area and take a tissue sample. Some surgeons will place a small metal chip at the biopsy site after taking the tissue sample so that future mammograms or ultrasounds will indicate where the biopsy was done. You may experience mild discomfort after these procedures, but will be able to resume normal activity within a day or so.

Definition/Overview An open biopsy is a procedure in which a surgical incision is made through the skin to expose and remove tissues. The tissue is then examined by a pathologist. In some cases, the lump may be removed along with surrounding tissue. Procedure During this procedure, you will either be given local anesthesia, local anesthesia with sedation, or general anesthesia. This will depend on the patient as well as the size and location of the lump. The surgeon will make an incision and remove the breast tissue sample and lump if necessary. After the sample is taken, the incision is sutured and a dressing is applied. Pain medication may be prescribed, and there may be soreness in the area, but generally patients can resume normal activity within a day or two.

Definition/Overview The sentinel lymph node is the first lymph node to which cancer cells are likely to spread from the primary tumor. Cells may appear in the sentinel node before spreading to other lymph nodes. Sentinel node biopsies are used to help determine the stage of cancer. Procedure During a sentinel lymph node biopsy, the surgeon removes one to five sentinel lymph nodes and sends them to a pathologist to determine if cancer cells have spread to them. If cancer is found in these lymph nodes, it is an indication that the disease has metastasized (spread). Before the procedure, the physician will numb the area and inject a blue dye or special tracer substance into the area around the original cancer site. The dye or tracer then moves to the first lymph node (sentinel node) that drains closest to the cancer site. The dye or tracer makes a map pattern of lymphatic fluid that shows where the cancer is likely to spread. That lymph node is removed, cut into thin slices, and viewed under a microscope. If the sentinel node is positive for cancer cells, additional surgery will be done to remove more lymph nodes. This is an outpatient procedure, and you may either be given general anesthesia, or a sedative to help you relax. The procedure itself takes 30 to 60 minutes. The biopsy site may be sore for several days after surgery, but you should be able to resume most activities within a day or two.

Bronchoscopy is a technique of visualizing the inside of the airways for diagnostic and therapeutic purposes. An instrument (bronchoscope) is inserted into the airways, usually through the nose or mouth, or occasionally through a tracheostomy. This allows the practitioner to examine the patient’s airways for abnormalities such as foreign bodies, bleeding, tumors, or inflammation. Specimens may be taken from inside the lungs. The construction of bronchoscopes ranges from rigid metal tubes with attached lighting devices to flexible optical fiber instruments with realtime video equipment.

Definition/Overview An esophagectomy is a procedure for the treatment of esophageal disorders including esophageal cancer. Procedure In this procedure, which is performed under general anesthesia, the part of the esophagus that contains the cancer and nearby lymph nodes are removed. The surgeon either attaches the esophagus directly to the stomach or replaces the removed part of the esophagus with a piece of intestine. Transhiatal Esophagectomy (THE) In some cases, a transhiatal esophagectomy (THE) may be performed. Incisions are made in the upper abdomen as well and the neck. Part of the esophagus is removed and part of the stomach is cut into a tube shape and is also removed. The stomach portion is then moved upward through the chest and connected to the small piece of the esophagus remaining in the neck. Laparoscopic Esophageal Surgery This procedure is minimally invasive and performed using small incisions through which specialized equipment and cameras are inserted. This surgery is not appropriate for all patients requiring esophageal surgery. Your physician will determine which surgical approach is best suited to your situation.

Definition/Overview A lobectomy is the surgical removal of one entire lobe (section) of a lung. The procedure may be performed when an abnormality has been detected in a specific part of the lung. The affected lobe is removed, and the healthy tissue remains. A lobectomy is preformed in early stage NSCLC patients (where cancer has not spread). A lobectomy is normally performed during a thoractomy. Procedure A lobectomy is an inpatient procedure and requires general anesthesia. During the procedure, an incision is made in the chest and the ribs spread apart to allow adequate viewing of the lung. Blood vessels and air tubes that supply the lobe being removed will be tied off with sutures. The lobe is removed and possibly a portion of a rib as well. The surgeon places a drainage tube or tubes in the chest to drain fluid that may build up in the chest. You may be given an epidural catheter in order to administer pain medication. You will normally spend the first day after surgery in the ICU (Intensive Care Unit), followed by a hospital stay of up to a week or more. You may need to receive oxygen in the hospital, and possibly after being discharged. The drainage tube will be removed after a few days or until fluid is no longer present. You will work with a respiratory therapist while in the hospital to aid in recovery, and will be given deep-breathing exercises and coughing techniques to help facilitate lung re-expansion and prevent pneumonia.

Lymphadenectomy is the surgical removal of one or more groups of lymph nodes. It is almost always performed as part of the surgical management of cancer. In a regional lymph node dissection, some of the lymph nodes in the tumor area are removed; in a radical lymph node dissection, most or all of the lymph nodes in the tumor area are removed. It is usually done because many types of cancer have a marked tendency to produce lymph node metastasis early in their natural history. This is particularly true of melanoma, head and neck cancer, differentiated thyroid cancer, breast cancer, lung cancer, gastric cancer and colorectal cancer.

Definition/Overview A thoracoscopy is a procedure that utilizes an endoscope to examine the pleura, lungs and mediastinum, and to obtain tissue for testing purposes. This procedure is done to assess lung cancer, take a biopsy for study, determine the cause of fluid in the chest cavity, introduce medications or other treatments directly into the lungs, and to treat accumulated fluids in the space around the lungs. This procedure is sometimes done as an alternative to a thoracotomy. Procedure This is an inpatient procedure using general anesthesia. The surgeon will make several, small incisions in your side and insert a thoroscope containing a tiny camera through one of the incisions. The surgical instruments are inserted through the other incisions. The lung to be examined is deflated to create a space between the chest wall and the lung. The patient breathes with the other lung as well as with assistance from a ventilator. Using the camera as a guide, the surgeon takes tissue samples and reinflates the lung. The surgeon closes the incisions except for one in which a chest tube is inserted to drain fluid and air from the incision site. The chest tube will remain for several days, and your hospital stay will typically range from two to five days. Medications for pain are given as needed, and you will have restrictions on lifting and other activities for several weeks.

Definition/Overview A wedge resection is the surgical removal of a small portion of the long along with the healthy tissue surrounding the lung. The procedure is performed to remove a lung tumor, or to aid in the diagnosis of lung cancer. Procedure This is an inpatient procedure and you will be given general anesthesia. The surgeon will perform a thoracotomy (an incision to open up one side of the chest), and will remove a wedge-shaped section of the lung. A chest tube may be inserted to allow draining of fluids that may build up in the lung. Your hospital stay will usually be around a week, and you will be given respiratory therapy during that time to aid in recovery, facilitate lung function and prevent pneumonia.

Some people hesitate to have their varicose veins evaluated and treated because they are concerned about the pain involved, recovery times, possible scarring and the expense. Fortunately, many of those concerns can be alleviated for most people. One of the most innovative treatments available to treat varicose veins is the minimally invasive VNUS Closure procedure*. Not only are pain and scarring minimized, but recovery times are typically much faster. Patients usually walk out the door shortly after the surgery. In fact, 98% of VNUS Closure patients would recommend the treatment to a friend or family member with similar leg vein problems. As far as the cost is concerned, if an individual has varicose veins severe enough to need surgical treatment, it is usually considered to be a medical condition – in many cases, this means that Medicare or insurance will cover the cost. VNUS Closure is a minimally invasive procedure that uses RF ablation to heat and shrink the vein in a controlled manner. Once the area is numb, a thin catheter (a tube even smaller than a coffee stirrer) is threaded into the damaged or diseased vein using a single needle stick. Proper placement of the catheter is assisted by the use of duplex ultrasound (similar to that which is used for obstetrical ultrasounds). The catheter delivers radiofrequency or microwave energy to the vein wall. This causes the vein to heat up, collapse and seal shut. After the diseased vein is closed, nearby healthy veins take over and empty the blood from your legs. Once normal blood flow is re-established, symptoms quickly improve. The procedure is less painful than some options and usually leaves the patient with little (if any) bruising, scarring or swelling. In addition, most patients can return to normal activities almost immediately. This is just one of many options available to patients today. Fortunately, the vascular specialists of Peoria Vein Center are experienced in many different procedures and can recommend the most appropriate treatment for each patient. Individuals that have a pacemaker, internal defibrillator or other active implanted device should have the vascular surgeon consult with the patient’s cardiologist and the implant’s manufacturer to determine if this procedure is a safe course of action.

Sclerotherapy is typically used to treat spider veins or small varicose veins.* During this procedure, a solution is injected into the vein using tiny needles. This solution (sclerosant) causes damage to the inside of the vein, which causes the vein to close. In time, the vein is absorbed into the body. The procedure usually takes place in our comfortable Vein Center facility using a local anesthetic. Using a series of tiny injections, a saline solution or other solution is injected into the vein. Or, in some cases, a tiny catheter may be inserted into the vein to allow the use of a foam sclerosant – if a catheter and foam are used, they will be guided by the use of duplex ultrasound. Some patients experience a feeling of minor burning and discomfort during the procedure. Repeat injections may be necessary to achieve the desired results. Depending on the size and number of veins being treated, the procedure is usually completed in 5 to 30 minutes. Before leaving the office, patients put on medical-grade compression stockings or elastic bandages as directed by their physician. Most patients are able to walk immediately after the procedure, but should refrain from strenuous exercise for approximately one week after treatment.

Topical Laser Treatment is usually the best option to minimize or eliminate tiny spider veins of the legs.* More women than men get spider veins, especially women taking birth control pills or hormone replacement therapy. This a cosmetic issue that is treated with a laser carefully focused on the problem areas. The tiny spider veins coagulate and shrink. Most patients notice an improvement in about a month. Typically, the main side effects are bruising and skin discoloration.
Mini-phlebectomy

Mini-phlebectomy


A mini-phlebectomy is an in-office procedure which is performed under local anesthesia. The procedure removes bulging varicose veins through very small incisions that usually require no stitches and virtually leave no scars. A mini-phlebectomy is used for the veins that are too large to be closed down with sclerotherapy. Usually, this procedure is very well tolerated and causes only minor discomfort while providing excellent cosmetic results. A patient can return to his /her daily activities (except for vigorous exercises) immediately following the procedure. A mini-phlebectomy is often performed in conjunction with a Laser Treatment.

Vein Ligation is a procedure that has been used for many years to eliminate larger varicose veins. This surgery is performed on an outpatient basis using regional or general anesthesia – most patients are able to go home later in the day. Typically, individuals are able to return to work in a few days (depending on their occupation), and get back to recreational activities in a few weeks. During the procedure, the surgeon makes several small incisions over the diseased vein and it is tied off (ligated). This option usually offers good long-term results, but can leave some scarring where the incisions were made. Although vein ligation can be performed for cosmetic reasons, it is usually done for patients that have large, bleeding, problematic (aching, swelling) or damaged veins; or open sores resulting from varicose veins. In some cases, sclerotherapy is recommended as a follow-up to vein ligation to treat smaller varicose veins. A qualified vascular surgeon can help determine the best procedure for each individual’s varicose veins. However, in general, the following people are not appropriate candidates for vein ligation: Pregnant women (although pregnancy can aggravate varicose veins, they should not be treated during pregnancy) Older adults with medical conditions that predispose them to risks associated with surgery Individuals with poor circulation in the arteries of the legs Those that have lymphedema, skin infections or blood clotting disorders

Vein Stripping continues to be an effective option for removing damaged varicose veins. When performed by a qualified vascular surgeon, it typically eliminates the diseased vein and improves symptoms. Using regional or general anesthesia, this procedure is done on an outpatient basis. During the surgery, the physician makes several small incisions – one at the groin, one at the knee and possibly a few others over the diseased vein. A special instrument is used to grasp the vein and pull it out. Blood then automatically reroutes to nearby healthy veins. This treatment option can be moderately painful and requires a recovery period of 5-10 days. However, it is a procedure that has been performed for many years and is usually effective. Some patients have expressed concern that they may need this vein as a graft for a heart bypass operation in the future. Because the vein is diseased, it would not serve as an adequate graft anyway. Therefore, it is important to remove the bad vein and resolve the problems it causes. Like vein ligation, stripping can be performed for cosmetic reasons, it is usually done for patients that have large, bleeding, problematic (aching, swelling) or damaged veins; or open sores resulting from varicose veins. In some cases, sclerotherapy is recommended as a follow-up to vein ligation to treat smaller varicose veins. An experienced vascular surgeon can help determine the best procedure for each individual’s varicose veins. Born out of Springfield Clinic’s Endocrinology department, The Diabetes Learning Center (DLC) was created to teach the newly—and not-so newly—diagnosed about their illness and its management.

The DLC works to achieve independence within the disease, educating patients and starting them on the path to self-management and independence. The DLC currently offers a series of four classes for patients. These courses will teach patients how to minimize the long-term effects of diabetes, manage their medications, adapt their lifestyles - including diet, exercise and healthy coping mechanisms - and be proactive in their diagnosis and disease.

Our staff

Springfield Clinic's Infusion Therapy Center is staffed by registered nurses who provide care that is overseen by an on-site physician. Score P4 is intended to be a safe, effective and supervised program in which physical therapists and athletic trainers will work with participants to focus on Preventing the risk of injury, helping achieve Peak Performance and helping to improve ability and confidence returning to Play. Specifically, this program is for active individuals and athletes returning from injury and targeting returns to their sports or usual recreational activities. Primary goals of the program will be to improve strength, speed, and power, particularly with higher level activity that may not be possible in a typical physical therapy environment or within the potential limitations or visit restrictions of insurance-based care.

The sports medicine program encompasses a team approach to meeting the needs of the community and provides a full range of medical and physical services to treat athletes of all ages and levels from junior high, high school, collegiate, professional to weekend warriors and fitness enthusiasts. Injuries can range from trauma and fracture management to overuse and preventative care.

As part of Springfield Clinic Orthopedics, the sports medicine specialists will coordinate the athlete’s care, including any imaging (x-ray, MRI, CT, etc), lab work, physical therapy or referrals to appropriate medical or surgical providers.
